Transition planning with young people is like being on a road trip toward the youth's chosen destination. In this workshop, participants will follow a well-planned route marked by intentional checkpoints to effectively assist transition-age young people with becoming adults. Each stop represents a critical stage in the journey -- clarifying key ideas, gathering essential resources, and applying proven strategies. Just as a driver relies on a map, fuel stops, and rest breaks to reach their destination efficiently, travelers will be equipped with the guidance needed to navigate planning with focus, direction, and confidence.

Participants will have a day of thoughtful discussion, guided exercises, and hands-on experience to better understand the needs of a transition-age youth, leaving with tools to better assist young people with navigating their trip and learning to effectively prepare for adulthood.
