Life Skills Assessment

An Oklahoma Successful Adulthood (OKSA) Guide

The Life Skills Assessment is designed to introduce youth to basic skills needed for success as an adult, and a tool to assist workers in determining the current life skill knowledge of the youth with whom they are working. The Life Skills Assessment must be completed before an OKSA eligible youth can begin to access funds and other OKSA services. The Life Skills Assessment should be completed within 30 days of the youth’s 14th birthday or within 30 days of the youth entering care, after the age of 14. For those youth who completed an assessment between the ages of 14 and 15, a new Life Skills Assessment must be completed within 30 days of the youth's 16th birthday. Please note that you will need the youth’s client ID number to complete the assessment.

Complete the Life Skills Assessment

  • Schedule a time to meet with the youth on or within 30 days of his/her 14th birthday, or within 30 days of the youth entering care after his/her 14th birthday. Explain to the youth that you will be assisting with the completion the Life Skills Assessment to determine his/her current life skills knowledge.
  • Prior to the meeting, print a copy of the Rights of Oklahoma Foster Youth form (04IL011E).
  • Meet with the youth to complete the Life Skills Assessment.
    • Review the Rights of Oklahoma Foster Youth form (04IL011E) with the youth and placement provider, and obtain signatures.
    • Provide the original to the youth.
    • Scan and save a copy signed by the youth, placement provider, and appropriate DHS or tribal representative in the client tab of the KIDS file cabinet.
  • Access the Life Skills Assessment at: https://www.surveymonkey.com/r/OKSAassessment
    • Read the instructions found on the opening screen and complete the survey as instructed.
    • Remain with the youth while he/she completes the assessment and utilize the “Comments or Notes” section as needed.
    • The Life Skills Assessment will be automatically sent via email once you complete all questions and select "Done."

      NOTE: If you complete the Life Skills Assessment on paper, please scan the completed assessment and email it to oksa@ou.edu. Your results report should arrive by email within 3 to 5 business days of submission.
  • You should receive a copy of the Life Skills Assessment Results Report from NRCYS via email within 30 minutes of completing the Life Skills Assessment online.
  • If you do not receive a copy of the results report within 24 hours, please contact oksa@ou.edu or 1-800-397-2945.
  • The National Resource Center for Youth Services (NRCYS) will save a copy of the assessment results in the KIDS filing cabinet, and will document the services in the OKSA Services screen.

Using the Life Skills Assessment Results Report

  • Review the Life Skills Assessment Results Report with the youth. It is imperative that you review the results directly with the youth. Others that should be included in the review and/or have copies of the results include the placement provider, worker(s), OKSA Specialists, and other significant adults who will be assisting the youth with his/her transition to adulthood.
  • Use the Life Skills Assessment Results Report to start a Plan for My Future with the youth.
