Plan for My Future

An Oklahoma Successful Adulthood (OKSA) Guide

The Plan for My Future is developed with the youth and with the help of supportive people and permanent connections identified by the youth. It is designed to assist the youth with a successful transition to adulthood. The initial Plan for My Future should be developed within 60 days of a youth’s 14th birthday or within 60 days of the youth entering care after the age of 14. The worker should review and update the plan with the youth and placement provider each month.

Initiate the Plan

  • Schedule a Family Team Meeting (FTM) to begin the initial Plan for My Future with the youth.
    • Participants at the FTM should include, at a minimum: the youth, the worker and the current placement provider.
    • The youth must also be given the opportunity to invite at least two supportive connections of his/her choosing to the FTM.
  • Prepare for the planning meeting with the youth.
    • Complete the Life Skills Assessment with the youth.
    • Discuss the purpose of the planning meeting.
    • Determine a location, date, and time for the meeting.
    • Invite supportive connections identified by the youth.
  • Meet with the youth and their supportive connections.
    • Discuss the purpose of the planning meeting.
    • Review the Life Skills Assessment Results Report and the 7 Key Elements to determine which elements the youth would like to begin with.

      NOTE: A youth may only be working on a few elements at a time, and may not have all 7 areas completed on their initial plan.
    • Provide the youth with a copy of the Plan for My Future document and provided in the assessment results email you received after the youth completed the Life Skills Assessment.
    • Use the instructions on the Plan for My Future document to begin the plan with the youth.
    • Use the youth's Plan for My Future during the development of the 90 Day Transition Plan (My Transition Plan), which must be completed 90 days prior to the youth's 18th birthday.

Document The Plan

  • Save a copy of the initial Plan for My Future in the KIDS File Cabinet.
    • Open the File Cabinet of the youth's case.
    • Select the "Client" tab and click on "Add."
    • Under "File Name," find the plan and attach the file.
    • Under "Description," enter: Plan for My Future and the youth's name.
  • Create an IL-ISP.
    • Select "Case Plan."
    • Select "Trmt/ISP."
    • Select "New."
    • Select "Gen. "Inf" tab.
      • Under "Plan Type," select a type based on the youth's custody status:
        • IL and Permanency for youth in permanent custody.
        • Independent Living Only for all other custody types.
      • Under "Plan Participants," select the youth's name.
      • Under "Permanency Plan," document the youth's current permanency plan.
    • Select "To Do" tab.
      • At least one "to do" should refer to the youth's Plan for My Future in the File Cabinet, and should indicate that the Plan for My Future is the working plan, will be updated monthly, and will be supplied to the court at each hearing as an attachment to the IL-ISP or progress report.
      • Select "Approval" and send for supervisor review and approval.
      • NOTE: The OKSA program only requires this abbreviated completion of an IL-ISP as a placeholder in KIDS, in order to show a current Plan for My Future is documented and saved to the File Cabinet. The OKSA Plan should then be attached to Progress Reports for court or as required for other professional documentation.
      • If the worker and/or supervisor intends for the IL-ISP (rather than the OKSA Plan) to be presented as part of a Progress Report for court or in another professional forum, the IL-ISP should contain more comprehensive information about the youth's plan and updated progress. This may be contingent upon individual supervisor, county, or jurisdictional preferences. If the IL-ISP must be utilized in these forums, we strongly suggest seeking training or technical assistance with completion of the IL-ISP. This will help to ensure best practices and outcomes for our youth.

Report

  • Report the plan and plan progress to the court.
    • Attach the youth's Plan for My Future to the youth's IL-ISP or progress report and submit to the court at each hearing.
    • Request the court finding of "IL services and supports have been provided" at each court review.
  • Schedule a FTM a minimum of every six months after completion of the initial plan to dicuss updates, progress, barriers and any additional needs of the youth. Subsequent FTMs should include the youth's worker and placement provider, as well as supportive connections of the youth's choosing.
    • Update the youth's Plan for My Future and save the updated plan to the KIDS File Cabinet.
    • Utilize the results of the FTM to update the court on the youth's plan, as well as OKSA services and supports provided to the youth.

Work the Plan

  • Prior to each monthly visit, print or download the Plan for My Future from the KIDS File Cabinet.
  • Review the plan with the youth during your monthly visit.
    • Update goals, steps, person assisting and end dates as needed.
    • Ask the placement provider to assist the youth in completing new tasks each month.
    • NOTE: Use the OKSA Guide for Documenting OKSA Services to help you document these tasks in KIDS each month.
    • Access Youth Development Funds to support the youth’s plan.
    • Provide updated copies of the plan to youth, placement providers, etc. after each visit.
  • Save the youth's updated Plan for My Future to the File Cabinet after each visit.
